A disabled person parking permit allows you to use designated accessible parking spaces. The permit you receive depends on whether your disability is permanent or temporary.
Permanent Disabled Person Parking Permit
In order to apply for a permanent permit, you will need the following:
- A completed Application for Disabled Person Parking Permit (Form HSMV 83039), signed by a certifying authority within the last 12 months
- A valid Florida driver's license or Florida identification card
There is no fee. The permit is valid for four years and expires on your birthday.
If your disability is too severe to visit an office for a driver's license or ID card, your certifying authority can complete the exemption section on Form HSMV 83039 so the permit can be issued without one.
Temporary Disabled Person Parking Permit
In order to apply for a temporary permit, you will need the following:
- A completed Application for Disabled Person Parking Permit (Form HSMV 83039), signed by a certifying authority within the last 12 months
- A valid driver's license or identification card
- $15 fee
A temporary permit is valid for the period set by your certifying authority, up to six months.
If your temporary disability lasts longer than six months, an additional permit can be issued. There is no additional fee if you apply within 12 months of your original permit.
Secondary Permit
A secondary permit may be issued to permanent permit holders who are certified on Form HSMV 83039 as a quadriplegic or as a frequent traveler. Secondary permits expire on the same date as your original permit.
Who Can Certify Your Disability
Form HSMV 83039 must be completed and signed by one of the following:
- A physician, osteopathic physician, or podiatric physician
- A chiropractor
- An optometrist (for legal blindness only)
- An advanced practice registered nurse, under the protocol of a licensed physician
- A physician assistant
Important: The certifying authority must include their license number and the state in which they are licensed.
