What to Bring
If you choose to come in person, you must bring two proofs of your new residential address.
Each document must:
- Show your full name and new Florida address
- Be dated within the last 60 days
Examples of acceptable documents include, but are not limited to:
- Household documents: Deed, mortgage, mortgage statement, residential lease/rental agreement, or property tax documents
- Florida voter registration card
- Florida vehicle/vessel registration or title
- Utility bills: Water, gas, electric, cell phone, or cable
- Automobile payment booklet
- Medical or health card/bill
- Homeowner’s or auto insurance policy or bill
- Employer documentation: Pay stub, W-2, or 1099 form
- Financial statements: Bank, credit card, or investment account
- Mail from government agencies
For CLP and CDL Holders
Customers applying for or holding a Commercial Learner’s Permit (CLP) or Commercial Driver License (CDL) must present:
- Two residential address documents from List A, or
- One document from List A and one from List B
All documents must have a Florida address in the driver’s name.
List A (CLP/CDL):
- Lease/rental agreement (6 months or longer)
- Florida voter registration card
- Florida vehicle/vessel registration or title
- Homeowner’s insurance policy or bill
- Utility bill (water, gas, electricity, cable — dated within 60 days)
- Telephone or internet bill (dated within 60 days)
- Property tax bill
- Mortgage deed for Florida property
If a document from List A cannot be provided, a Declaration of Domicile filed with the Clerk of the Circuit Court may be submitted along with one document from List B.
List B (CLP/CDL):
- W-2 or 1099 form
- Bank or credit card statement (dated within 60 days)
- Military orders or documents
- Pay stub (dated within 60 days)
- Government-issued mail or official document (dated within 60 days)
