To apply for a Florida Identification (ID) Card, you will need to provide the following documents:
Required Documents
Please bring the following original documents:
- An original or certified U.S. birth certificate, or a valid U.S. passport
- Your Social Security card
- Two proofs of residential address from two different sources, dated within the last 60 days
For children, the proof of residential address may be in a parent’s or legal guardian’s name.
If your name has changed since birth, you must also provide original or certified legal name change documents, such as a marriage certificate or court order.
Fees
The fee for an original, renewal, or replacement Florida ID card is $31.25, regardless of the applicant's age.
Card Validity
- Children ages 5 through 14 are eligible for a Florida ID card. These cards are valid for 4 years.
- Customers age 14 and older receive an ID card that is valid for 8 years.
