If you are 80 years of age or older and are not eligible to renew your driver license online, you are required to pass a vision test before your renewal can be processed. You have two options for completing this requirement.
Take the Vision Test at Our Office
You can complete the vision test at any of our office locations at no additional cost during your renewal visit.

Have Your Doctor Administer the Test
You can have a Florida-licensed medical doctor, osteopathic physician, or optometrist administer your vision test. Once you pass, you or your doctor must submit the Mature Driver Vision Test form (HSMV 72119 S) before your renewal can be processed.
The form is printed on the back of the renewal notice mailed to you, so you can take it directly to your doctor.
If the test reveals a condition that requires a referral to an eye specialist, such as a Florida-licensed ophthalmologist or optometrist, you will also need to submit a completed Report of Eye Exam Form (HSMV 72010).
If driving is no longer a safe option for you, you may surrender your driver license and apply for a Florida ID card instead.
